Service/Procedure	Provides extensive rehabilitative services to patients who require a hospital level of care.  Rehab is provided in a realistic “Easy Street” environment consisting of a car, gas pump, grocery store, bank, football stadium and pouch venues.
How it Works/ What it is	Easy Street Rehab Unit is a 17 bed unit which serves patients with a multitude of diagnoses.  The most common rehabilitation diagnoses include stroke, orthopedic and neurological conditions.  Most patients are admitted directly from a hospital’s medical/surgical unit, but patients can be admitted from any level of care, including home.  Rehabilitation nursing, physical therapy, occupational therapy and speech therapy are provided.
Benefit to Patient	Patients receive a 24 hour a day hospital level care with an intense rehab program coordinated by a multi-disciplinary team approach designed to improve the patient’s ability to function.  The use of the “Easy Street” environment allows patients to transition back into their environment at their highest functional level possible.
Benefit to Referring Physician	While on the unit, a patient’s rehab care is managed by a physician who has training and experience in rehabilitation.
